Missing October issue 216

I'm TAS subcriber acc #00000085813. I have not receive (missing issue) my October edition (issue 216). I already receive Nov and Dec edition. Could you please resend my October Issue 216.
I tried to contact you using "contact us" page in the webside bit it lead to error
